Functional Primary Care Membership plans will have a one time $350 enrollment fee (initial visit fee + 1st month membership)

Includes (1) Primary Care visit per month (up to 40 min) , 5% discount on Supplements, GLP1 medications, Peptides, IV hydrations. 

Discount Labs through QUEST DLO. Also available, Functional testing starting at just $249.
